Evaluation Resources

Over
the past five years, Innovation Network (InnoNet) has worked
closely with the Annie E. Casey Foundation and the KIDS COUNT
Network. During this time, the Network, in conjunction with
InnoNet, has produced numerous tools and instruments to help
organizations gather important information to evaluate the
work that they do. All of these tools and instruments have
been pilot tested by the Network and have proved repeatedly
to be beneficial and worth the time invested. The tools have
been designed with the ability to adapt more specifically
to the customized needs of your organization. We encourage
you to edit and add components to the tools, as you see fit.
For your use, blank copies of the forms found in each section
of this Toolkit are located in the appendix.

Self Assessment Section (pdf) -
Launched
in 2000, The Self-Assessment Tool began as a product
of collaboration between KIDS COUNT Network members
and the staff at InnoNet. The tool was designed for
KIDS COUNT Network members to assess the components
of individual KIDS COUNT projects for planning purposes.The
Self-Assessment Tool is divided into five sections that
represent the major program components originally identified
by the KIDS COUNT Network
- Request
Tracking Section (pdf) -
KIDS
COUNT members interact with constituents in a variety
of ways. These interactions are valuable opportunities
to gather information that can help
create KIDS COUNT products, marketing, and outreach
work. One way to keep track of interaction with constituents
is the Request Tracking Form and the
Request Tracking Spreadsheet.
- Workshop,
Training and Presentation Evaluation (pdf) -Trainings,
workshops, and presentations are an important way to disseminate
information about KIDS COUNT issues, as well as an opportunity
to reach new KIDS COUNT users. The effectiveness of your
presentation has a direct impact on how successfully your
audience retains the information, and how your attendees
will use KIDS COUNT information in their own organizations.
The Evaluation Form is a survey that
is designed to be given to your participants after any
type of presentation, in order to gain immediate feedback
on how successful the presentation was. The Evaluation
Form will allow you to capture valuable presentation feedback
that can help your organization reach audiences more effectively
in the future.

Feedback Survey Section (pdf) - Every
State KIDS COUNT grantee puts a tremendous amount of
effort each year into their data books. Wouldn't it
be helpful to know who is reading the books and what
they like or dislike about them? Additionally, do they
have any suggestions or wishes about the books? These
important questions can be answered by using a simple
User Feedback Survey.
- Media
Tracking Section (pdf) -Media
coverage is critical to the KIDS COUNT projects' ability
to disseminate messages and gain support for important
issues that affect children and families. KIDS COUNT projects
vary in size and scope, and therefore have different levels
of interaction and exposure in the media. Because media
exposure is such an important tool for gaining attention
and achieving progress on KIDS COUNT issues, evaluating
KIDS COUNT coverage in the media is critical. The Media
Tracking Form is designed to systematically track how
effectively and accurately KIDS COUNT receives coverage
in the media.
